Follow-up
Follow-up appointments may be recommended at the provider's discretion for a variety of reasons, including lab result reviews, medication management, referrals, and other necessary evaluations, to ensure that your health plan remains on track and your care continues to meet your needs. Some chronic medical conditions require regular follow up (every 3-6 months) for optimal management.

Sick / Problem Visit
If you are experiencing symptoms of a contagious illness (e.g.: coughing, sneezing, fever, sore throat, and congestion/ runny nose) we ask that you please wear a mask while at our office to limit the exposure to our staff and other patients in the facility.

Pre Op Clearance
Pre-op clearance appointments may be required by your surgeon. This does require an in-person visit with your provider and can not be combined with another type of appointment. During this visit your provider will review your medical history, medications, supplements, and planned procedure to make sure you, your surgeon and your anesthesiologist are fully prepared for a successful surgery and recovery.
